How to enable Centrix for Driver License Checks

If you would like to switch on the Centrix SmartID integration to perform 3rd party identity verification of drivers licenses in Connectworks please download the authority form below and return it to support@connectworks.com

We will then advise when the function has been enabled in your account, this usually takes around 48hrs.

How to enable Centrix for Passport Checks

In addition to completing & returning the above Centrix application form , the DIA requires an additional application form (below) as well as a copy of the firm's privacy policy along with the application to enable passport checks.

If there is not already a privacy policy implemented, a privacy policy generator is available here.

Please return all forms to support@connectworks.com. We will advise when the function has been enabled in your account, this may take up to 30 days.

    Important
In order for the DIA to process your application the form must be:
    1. Completed electronically
    2. Only the signature can be hand written
    3. All questions must contain a response
